Sunken Eyes, Under-Eye Bags and Dark Circles

The shadow that stays regardless of sleep.

Under-eye skin is very thin, among the thinnest on the face, so whatever sits beneath it shows through. A shadow that is still there after a full night's sleep usually comes from structure: a tear trough that has deepened, or thin skin letting vessels read through. Concealer evens the tone, but it cannot change what sits underneath.

CONCERN & CAUSE

Where That Under-Eye Shadow Comes From

Across a room, a hollow, a vessel showing through and an eye bag can all look like the same dark circle. A bag may be a small fat pad pushing forward or simply fluid, and each cause answers to a different treatment. Your consultation identifies which one applies here, and the treatment follows from that. Under-eyes that sink suddenly alongside illness belong with your physician first.

01

A Hollow Where Support Used To Be

The fat pad in the mid-cheek slides down over the years and the tear trough deepens into a groove. Overhead light falls straight into it and the eye looks sunken. Significant weight loss empties the same space faster, and smoking speeds the collagen loss underneath. Deep-set eyes show some version of this from the twenties on.

02

Vascular show-through

Fine veins and the muscle around the eye sit close to the surface with almost nothing covering them. In certain light they read blue or purple straight through the skin. Fair, delicate skin shows it most.

03

Pigment changes

Sometimes the darkness really is pigment. Post-inflammatory marks from eczema or years of allergy rubbing appear around the orbit, and deeper inherited tones show in the same place. Pigment drives fewer under-eye shadows than people assume, and brightening only helps when it is the true cause.

04

Crepey Skin And Fine Lines

Thin, crepey skin scatters light and exaggerates whatever sits beneath it. Sun exposure and steady collagen loss thin this area over the years, and squinting adds to it. Once the texture goes papery, every shadow underneath reads darker than it is.

Targeted Protocols for the Under-Eye and Tear Trough

An under-eye consultation is largely diagnosis. Catherine Curtin, MSN, FNP-C has worked with PRP and PRFM for about 15 years, and she examines the area in different lighting to establish how much of your shadow is hollow, vessel, pigment or skin quality before recommending anything.

Under-eye filler for hollows

Hyaluronic acid gel is placed deep at the bone in the tear trough, in fractions of a syringe. It replaces lost volume, so the skin above is supported and the hollow stops casting a shadow.

Microneedling opens fine channels in the under-eye skin, and platelet-rich plasma is applied into them to drive collagen remodeling. Around the eye the needle depth stays shallow.

Looking After This Skin At Home After Treatment

Treatment results hold longer when the skin around your eye is cared for daily. This area is thin and reacts easily, so the routine below stays gentle and consistent. Keep it up between visits.

FAQ

Common Questions About
Under-Eye Treatment

Filler placed in the tear trough often lasts longer than filler in expressive areas, because this part of the face barely creases. How long a result holds depends on the product used and how much was placed. The area is reviewed at a follow-up visit, and more is added only if the structure needs it.

Only when the darkness is a hollow throwing a shadow. If it comes from vessels under very thin skin, filler will not correct it and can make the area look worse. Separating the structural part from the surface part is most of what your consultation is for.

Most people describe pressure under the eye more than anything sharp. The area is numbed first, and most of the fillers used here contain lidocaine. Mild swelling or bruising in the days after is more common than any discomfort during.

It is a bluish cast that can appear when hyaluronic acid sits too close to the surface under very thin skin. Light scatters through the gel and the area looks blue. It can be dissolved with an enzyme. Placing the product deep against bone, in small amounts, is how it is avoided in the first place.

Usually not. A bruise here is small, and most people go back to work the next morning and no one says anything. Puffiness can run a few days. Pausing aspirin, ibuprofen and fish oil beforehand keeps bruising down, and skipping alcohol after helps too. Never stop prescription medication without your prescriber.

Sleep, salt, alcohol, sinus congestion and allergies all shift how much fluid sits in the area, and fluid makes a hollow look deeper than it is. A true structural hollow looks the same on your best-rested morning. That pattern is what the in-person assessment separates.

Skin first, structure after. PRP or microneedling with PRP strengthens a thin surface, and Agnes RF is the tool when the bag is a small fat pad pushing forward. Filler comes last, and only if a hollow is still there at that point. The spacing between steps is mapped at your consultation.

It depends on what your eyes need. Dermal filler is $750 to $900 per syringe, and the under-eye rarely takes a full one. Microneedling is $400 on its own, though around the eye it is almost always combined with PRP or exosomes, which is $200 to $500 more. Your consultation is complimentary, there is no deposit, and you will know the expected cost before you decide on anything.

A good eye cream helps with puffiness and fine lines, and it protects this skin daily alongside in-office treatment. It cannot rebuild lost volume or correct vessels showing through thin skin. Home care is matched to the in-clinic plan at your consultation.

No. Many under-eyes do best with skin work first, and filler is not right for every first visit. Overfilling here is hard to hide and slow to leave. Where surgery is the better option, the consultation covers that. Eyes that sink suddenly with illness or after an injury go to your physician first.
