Relaxing those muscles means they contract with less force, so the skin above them pleats less. The usual targets are the nasalis, which pulls the nose into bunny lines, and the orbicularis oris, which purses the lips into fine vertical lines. Softening begins within the first few days and reaches full effect in about two weeks. It does nothing for a deep nasolabial fold, which comes from lost volume and is treated with dermal filler.

A single unit around the mouth changes how the lip lifts and how words form, so fewer injection points are used here than on the forehead. Each point is chosen against the way your own mouth moves, and the dose at each one is small.

Botox cannot be dissolved once placed; it wears off gradually. Around the mouth that means a small first dose into the few muscles creasing your skin, so speech and lip movement stay normal while the lines soften.

Some mouths need a different treatment. Lines etched into thin skin respond to resurfacing or filler more than to muscle relaxation, and Catherine discusses that recommendation at the consultation.

What Patients Ask About Treating Smile Lines

The questions that come up in almost every smile line consultation, answered here so you arrive already knowing most of it.

It is priced by the unit. The muscles around the mouth and nose are small, so they take very few units, and dosing is kept conservative here. A unit is $15.50, and Beauty Bank members pay less (figures on the Beauty Bank page). The likely unit range is given at your complimentary consultation, and there is no deposit.

Botox softens the crease, and it does not remove it. The line still forms when you laugh, less deeply, and the skin recovers faster afterward. Patients often describe the area around the mouth looking less tired in photographs. Lines etched into the skin at rest need filler or resurfacing to change further.

Dermal filler. It is placed to rebuild the cheek support above the fold as often as in the fold itself, using Restylane or an RHA filler at $750 to $900 per syringe. Botox is not used to fill a fold. The consultation is complimentary and there is no deposit.

Three to four months for most patients. Around the mouth the dose is light and these muscles work constantly through talking and eating, so the softening can fade sooner. Tell Catherine when movement returns, and the timing of the next visit is set from that.

The treated muscles start to relax within the first few days, and the creasing softens as they do. Most of the change is there by the end of week one, and it reaches full effect in about two weeks. That is when Catherine wants to see you back and check it properly.

Too much dose around the mouth affects how the lip lifts and how words form. For that reason only the few muscles creasing your skin are dosed, and the lip elevators that raise your mouth into a smile are avoided. The first dose is kept small, and units can be added at the two-week review.

Perioral lines are the fine creases around the lip border, often nicknamed lipstick lines. Small, conservative doses can soften the muscle-driven puckering when your anatomy suits it, and your speech stays completely normal. Thin, deeply etched skin is a different problem, and Catherine would point you toward resurfacing for that.

Usually yes, and most patients do. The upper face takes a firmer dose than the mouth does, so Catherine treats them as two separate decisions inside one appointment. If it is your first time in the lower face, she sometimes stages it so she can read the mouth on its own.

Whichever matches the cause. Creasing that appears only when you smile is muscle-driven, and Botox softens it. A groove present when your face is still comes from lost support beneath the skin, and filler rebuilds that support. Many mouths need some of each. Catherine decides after examining how your mouth moves, and the assessment visit is complimentary with no deposit.
